Complete this form to start your onboarding as an independent sales representative. It takes about ten minutes.
Before you begin
Have these ready.
- 1A photo of your driver’s license — front and back. Phone photos are fine as long as all four corners and the text are readable.
- 2Your team lead’s name. If you are not sure which team you are on, choose Unknown and we will sort it out.
- 3About ten quiet minutes. You will review and sign the 1099 independent contractor acknowledgements and the sales representative agreement, so read them rather than skim.
One more thing
After you submit, watch your email. You will receive a separate invitation from Gusto to provide your W-9 and direct deposit details — you are not fully onboarded until that is finished too.
